What are risk forms?

Risk forms are dynamic response forms that users can create, customize, and publish in 6clicks. Part of the Risks module—alongside Risk Registers, Risk Libraries, and Risk Reviews—this feature is designed to make risk submission seamless and accessible across your organization. Beyond risk and compliance professionals with platform access, any employee can now complete a risk form to directly raise risks.

How risk forms work in 6clicks

Beyond being easy to use for employees submitting risks, risk forms are also highly configurable for administrators and risk managers. From the platform, users can:

  • Set form details: Define the form namedescriptionowner(s), and the default workflow stage that will be assigned to any risks captured through the form.

  • Customize form fields: Add fields to capture key risk details such as Risk NameDescriptionRisk DomainDate, or other custom fields. The Risk Name is mandatory by default, and you can mark other fields as required and reorder their sequence.

  • Configure risk assessment fields: Users can also enable risk assessment within the form and:

    • Choose the appropriate label (Current, Planned, Residual, etc.)

    • Add risk assessment fields such as Assessment TitleDateLikelihood, and Impact

    • Define mandatory fields and reorder their sequence

  • Share via public link: Once you have configured the form, click Publish to generate a publicly accessible URL that non-risk or compliance teams can use to submit risks.
